At Luso Care Centre, we work closely with healthcare professionals, local authorities and commissioning teams to provide safe, appropriate placements for adults with complex care needs. Our purpose-built home offers a supportive environment where individuals can receive specialist nursing, dementia and mental health care delivered by an experienced multidisciplinary team.

We understand that making a referral involves trust and careful consideration. Our team works collaboratively with social workers, hospital discharge teams, case managers, NHS Continuing Healthcare assessors and local authority professionals to ensure every placement is appropriate, well-planned and supported by a clear care pathway. From the first enquiry through to admission and ongoing review, we aim to provide a smooth and responsive referral process that prioritises the individual’s wellbeing.

Our home has been thoughtfully designed to support residents with a range of complex conditions, offering structured environments, specialist nursing oversight and personalised care planning that adapts as needs evolve.

Making a Referral

Referrers are welcome to contact the team at Luso Care Centre to discuss a potential placement and share the individual’s current care needs. During this first conversation, we gather key background information about the person’s health, support requirements and current circumstances. This helps us determine whether the home is likely to be an appropriate environment and ensures the referral can move forward in a timely and informed way.

Following the initial enquiry, our clinical team reviews the referral information and arranges a comprehensive assessment. Wherever possible, this assessment is carried out in the individual’s current setting, which may be a hospital ward, their home or another care environment. The assessment allows our team to understand the person’s physical health, cognitive needs, emotional wellbeing and day-to-day support requirements so that we can determine how best to support them.

When a placement is agreed, our team coordinates the admission process to ensure the transition into the home is as smooth and supportive as possible. We liaise with the referrer, healthcare professionals and family members to plan the move carefully, ensuring that all relevant information, medication and support arrangements are in place. Our aim is to help each new resident settle into the home comfortably while maintaining continuity of care from the moment they arrive.

Complex Nursing Care

For individuals living with advanced health conditions or multiple long-term illnesses, Luso Care Centre provides round-the-clock nursing care delivered by experienced registered nurses.

Residents may require ongoing clinical monitoring, medication management and specialist interventions to support complex physical health needs. Our nursing team works in partnership with GPs, community healthcare teams and specialist services to ensure care remains coordinated and responsive.

By combining clinical expertise with a supportive residential environment, we aim to maintain comfort, safety and quality of life for residents whose care needs require a higher level of medical oversight.

Specialist Dementia Care

Luso Care Centre supports individuals living with dementia who require both residential care and ongoing nursing oversight. Our team has experience supporting residents whose cognitive changes may affect behaviour, communication and daily functioning.

Care is delivered within a structured and supportive environment where consistent routines and familiar surroundings help reduce anxiety and promote stability. Individual care plans focus on maintaining dignity, supporting orientation and enabling residents to remain engaged with everyday life wherever possible.

Mental Health Care

Our Care Centre also provides residential care for adults living with complex or enduring mental health conditions who require a stable and supportive environment.

Care plans are designed to reflect each individual’s needs, helping them manage symptoms, maintain routines and feel secure within the home environment.

Luso Care Centre accepts referrals for adults who require specialist residential or nursing care due to complex health conditions. This includes individuals living with dementia, neurological conditions, complex physical health needs, acquired brain injuries or enduring mental health conditions. Our team reviews each referral carefully to ensure that the home is able to provide the appropriate level of care and support.

We understand that many referrals require timely decisions, particularly for individuals who are ready for discharge from hospital or who require urgent placement. Our clinical team aims to review referral information promptly and arrange an assessment as quickly as possible. In many cases, assessments can be organised within a short timeframe depending on the availability of clinical information and the individual’s current setting.

When making a referral, it is helpful to provide as much relevant information as possible about the individual’s health needs, current care requirements and background history. This may include clinical summaries, risk assessments, current care plans, medication details and any relevant reports from healthcare professionals. Providing this information enables our team to complete a thorough assessment and determine the most appropriate care approach.
